Registered Nurses-Evening/On Call

RN would be responsible for patient visits in the homecare and hospice programs early evening.  Responsible for carrying pager for PRN patient visits through out the night.  Guaranteed hours.

• Current RN licensure required
• Home Health care work experience required
• Must possess and maintain current CPR certification
• Valid Driver's License, own transportation with current auto insurance
• Computer skills helpful.
• Possess strong written and verbal communication skill. 

Physical Therapist

The Physical Therapist (PT) is responsible for the evaluation, planning, implementation, treatment and documentation of patient care for home Physical Therapy.

• Previous physical therapy experience in an acute care facility required
• Home health care work experience preferred
• Current licensure required
• Must possess and maintain current CPR certification.
• Computer skills helpful
