Registered Nurse-Full Time

The RN is responsible for the patient assessment, implementation of the plan of care and documentation of the homecare patient.

  • Current RN licensure required
  • Home Health care work experience required
  • Must possess and maintain current CPR certification
  • Valid Driver's License, own transportation with current auto insurance
  • Computer skills helpful.
  • Possess strong written and verbal communication skill. 

Medical Social Worker-Per Diem

  Medical Social Worker provides social work services in accordance with the physician's plan of

  treatment.   Accepts clients referred by the client care manager.  Develops a social service care plan

  including making referrals and follow-up with families in need of support services.  Assesses Medicaid 

  eligibility, processes Medicaid applications and maintains contact with the Division of Human Services. 

  Counsels clients and families concerning the facilitation of the medical plan of treatment Develops, 

  prepares and maintains individualized client records with accuracy timeliness and according to policies.

  • Current Social Worker license in the State of NH
  • Valid Driver's License, own transportation with current auto insurance
  • Home Health care work experience strongly preferred.
  • Computer skills helpful

Physical Therapist-Part Time/Per Diem

The Physical Therapist (PT) is responsible for the evaluation, planning, implementation, treatment and documentation of patient care for home Physical Therapy.

  • Previous physical therapy experience in an acute care facility required
  • Home health care work experience preferred
  • Current licensure required
  • Must possess and maintain current CPR certification.
  • Computer skills helpful

 

Occupational Therapist-Per Diem

The Occupational Therapist would be responsible for the Evaluation, planning, implementation, treatment and documentation of patient care for Occupational Therapy

  • Minimum of 1 year of occupational therapy experience in acute care.
  • At least two years of home health care work experience preferred.
  • Current license to practice as a Occupational Therapist in the State of NH.
  • Must possess and maintain current CPR certification.
  • Computer skills helpful  

Certified Occupational Therapy Assistant-Per Diem

The Occupational Therapy Assistant carries out treatment, implementation and documentation of client status for home health clients under the direction of the registered occupational therapist.

  • Minimum of one year of occupational therapy assistant experience in acute care
  • At least two years of home health care work experience preferred.
  • Must possess and maintain current CPR certification.
  • Current license to practice as a Certified Occupational Therapy Assistant in the State of NH.
  • Computer skills helpful.

Speech Language Pathologist-Per Diem

Speech and language pathologists (SLP) work closely with people of all ages with varying degrees of voice, communication or swallowing problems. This involves the assessment of clients' problems and needs and the development of patient treatment plans.

  • Master's degree in Speech Language Pathology required
  • Home care experience preferred
  • Current licensure required
  • Computer skills helpful
