Hospice Program

Hospice Philosophy of Care

Hospice provides care and support to individuals who have a life threatening illness and who have decided to emphasize quality of life for time remaining and are not seeking curative treatment. Care is provided to individuals and their families with an emphasis on quality of life, comfort, dignity and respect. It is a team approach to care emphasizing physical, emotional and spiritual support.

Who is eligible for Hospice Services?

  • Diagnosed with a life threatening illness
  • Patient choosing not to seek curative treatment
  • Physician can certify that if the disease runs an expected clinical course, person may die in 6 months.

Do I need a referral from my Doctor?

  • You do not need a referral to consult with a Hospice Professional to seek information and understanding about Hospice Care. You will need to be referred by a physician for Hospice Care to be initiated

Where can Hospice care be provided?

  • Patient’s home or home of person caring for patient
  • Nursing Home
  • Group Home

Who provides Hospice Care?

  • An interdisciplinary team of professionals with a commitment to providing patient and family/caregiver support that will promote physical, emotional and spiritual comfort at the end of life.

  • Bereavement Support
  • Physical, Occupational, and Speech Therapy
  • Medical Director Consultation
  • Pain and Symptom Management

How is Hospice Care paid for?

  • Medicare Hospice Benefit
  • Private Insurance Plans
  • Private Pay

What if I don’t have any insurance?

  • No person is denied Hospice Services due to financial circumstances. Sliding fee scales are available. Hospice relies on donations and Memorial contributions to make services available to the communities it services.

Advance Directives

Making decisions about medical care is not always easy. At some point you may not be able to make your own health care decisions. That is why it is important to talk about your feelings and beliefs with your family/loved ones before critical decisions must be made.
