Home Health Care

VNA-Hospice of Southern Carroll County and Vicinity, Inc. is a not-for-profit Medicare certified home care provider licensed by the State of NH to provide comprehensive health care services to patients in their own home. Our staff works closely with individuals and their physicians to develop an individualized plan for care to help reach specific goals and to maintain their comfort and independence. We promote health and independence through education and skilled care services.

Home care may be appropriate when:

  • Recovering from illness, accidents or surgery
  • Suffering from cardiac or respiratory problems
  • Coping with a chronic illness
  • Managing complex medications regime
  • When learning care giving skills
  • Following a recent hospitalization

Home care services may also be needed for:

  • Home safety concerns
  • Health education
  • Nutrition counseling
  • Case management
The following services are available:

Registered Nurses provide skilled care, and education to clients in need of disease or medication management, wound care and infusion therapy.

Licensed Nursing Assistants provide personal care to clients who need assistance with personal care and activities of daily living.

Homemakers provide house-keeping, laundry, and errands .


Rehabilitation services Our Rehab Services department consists of Physical Therapy, Occupational Therapy and Speech Therapy. Therapists help clients achieve safety and independence in their home and community.

Pre-hab therapy visits available prior to joint replacement surgery.

Our Physical Therapy Department offers a Falls Prevention Program both in the clinical setting and through community outreach.

Something new in our Speech Therapy department is VitalStim Therapy (www.vitalstimtherapy.com), which is the only neuromuscular electrical stimulation that is backed by compelling clinical data and approved by the FDA to treat swallowing problems (dysphagia). Our Speech Pathologist is certified to perform this treatment. This therapy is available only in certain areas.

Medical Social Services assist both Home Care and Hospice clients with long term care issues, advance directives, caregiver stress, and financial concerns related to illness.

Volunteers are available in the home care program to visit patients for respite care, companionship and errands. (Click here for more information about VNA volunteers)

Home-Med Monitoring is long distance monitoring of vital signs, weight, oxygen levels from a central station in the agency office. Monitored by a nurse, when abnormal results are identified, the patient is called and a home visit is made if needed.

Home safety evaluations may be provided in the home to assess the need for modifications and/or additional services or supports in the home.

Services may be paid for by:

Medicare, Medicaid, HCBC, Private insurances, Self pay or on a Sliding fee scale

Referrals for care may be made by the physician, hospital, patient, family.
