About VNA

VNA-Hospice is located on Main Street in Wolfeboro, NH, the Oldest Summer Resort in America.  We are an integral part of the communities we serve and have been providing home health care since World War I
(History of VNA-Hospice). The Agency provides Home Health Care, Hospice and Palliative health care and Maternal Child Health Care/Community Services to residents of Alton, Brookfield, Effingham, Freedom, Moultonboro, Ossipee, Sandwich, Tamworth, Tuftonboro, Wakefield, Wolfeboro and other communities upon requests.  We have a dedicated staff of over seventy-five people whose only mission is to provide a comprehensive range of quality home health care to all of our clients.  We are available twenty-four (24) hours per day, seven (7) days a week. 

VNA-Hospice is an independent, not-for-profit (501(c)3) Agency that is licensed by the State of New Hampshire.  Home Health Care and Hospice Programs are certified by the Centers for Medicare/Medicaid Services. The Agency is governed by an all-volunteer  Board of Directors, who are elected and represent each town within our service area.

Home health care is the fastest growing industry in health care.  It has been shown to be an effective and efficient alternative to institutionalized health care. People want to be in their own home and they appreciate being able to receive high quality, compassionate care from a team of professional caregivers. The goal of VNA-Hospice is to maintain every client in their home setting when there are health and/or social needs, which interfere with their independence. Our in-home services are always provided under the direct orders of each client’s physician.

In 2002, the Agency took a major step toward its mission of providing comprehensive health care to our clients by becoming a HomMed Partner. HomMed is one of the leading companies to develop a telehealth monitoring system for in-home use. It allows our professional staff the ability to monitor a client’s health data on a daily basis. When a problem occurs, the staff, in collaboration with the client’s doctor, is often able to implement early and effective interventions that allow the client to remain at home. VNA-Hospice has over thirty (30) monitors and is able to provide telehealth monitoring to any resident within our territory. 

Services are billed to Medicare/Medicaid, to other insurance providers that the Agency contracts with or to private payers. While payment for services is always expected, a sliding fee scale can be used to determine individual payment when necessary.
